This service is designed to work with youth ages 17-21. Included is a comprehensive evaluation of skills and the availability of an independent living skills class.
Although close supervision and assistance is provided by program staff, we recognize
that youth this age will experience both success and failure while in placement.
Youth will be given every opportunity to learn from their mistakes as well as
their successes. The program serves both male and females, including mother and
child placements, pregnant teens and youth with handicaps. Each case is evaluated
and a specific Individual Service Plan is developed.
Youth who successfully complete the program may elect to remain in their apartments. Those who wish to relocate will receive assistance in doing so.
* ability to provide supervision to youth in a scattered site apartment environment
* ability to assess and develop a plan of instruction for independent living skills
* develop an Individual Service Plan within 30 days
* regular, detailed reporting of progress toward stated outcomes
